Spare part surgery principles dictate that whenever possible tissues should be salvaged and used for reconstruction. Failed brachial plexus surgery with a chronic clavicle non-union is a rare and challenging reconstruction. An ipsilateral pedicled osteocutaneous flap from the forearm was harvested at the same time as transhumeral amputation and used for reconstruction of the clavicle defect and resurfacing of the painful brachial plexus nerves, reducing neurostenalgia symptoms.
